When it comes to heart failure care, GPs play a central role: they are the first point of contact for patients, particularly for early diagnosis, long-term care and coordination of treatment.

If you integrate these practices into your telemonitoring network as primary care providers (PCPs), everyone involved benefits:

  • Efficient division of labour: The GP practice provides primary care, while you as the TMZ contribute telemedical monitoring and your cardiological expertise.

Good to know:

As a telemedicine centre, cardiology practices usually provide the indication for telemonitoring themselves and then take over the monitoring - they then act as a Primary treatment operator (PBA).

However, a GP practice can also take on the role of PBA and carry out telemonitoring together with you - with a clear division of tasks and close collaboration (see infographic).
