GP practices as referring physicians

When it comes to heart failure care, GPs play a central role: they are the first point of contact for patients, particularly for early detection, long-term care and coordination of treatment.

If you integrate these practices into your telemonitoring network as primary care providers (PCPs), everyone involved benefits:

  • Efficient division of tasks: The GP practice provides primary care, while you as the TMZ contribute telemedical monitoring and your cardiological expertise.
  • Stronger referral relationship: Close cooperation strengthens your relationship with GPs and positions you as a reliable partner.
  • Better patient care: Together, you can significantly increase the reach of telemonitoring and sustainably improve the care of heart failure patients.

Good to know:

As a telemedicine center, cardiology practices usually provide the indication for telemonitoring themselves and then take over the monitoring - they then act as a Primary treatment agent (PBA).

However, a GP practice can also take on the role of PBA and carry out telemonitoring together with you - with a clear division of tasks and close collaboration (see infographic).

Clinics as referring physicians

Hospitals also regularly treat patients with decompensated heart failure - precisely the patient group that benefits from telemonitoring. Through structured discharge processes, digital interfaces and shared care pathways, you as a telemedicine center can take over follow-up care, reduce readmissions and support the hospital in meeting quality indicators. In this way, your practice positions itself as an extension of inpatient care and as a central partner for telemedical care in the region.
