At the end of 2025, ZDF reported on the Telementor COPD study at LungenClinic Großhansdorf (et al.) and the care model behind it:
Target group
COPD patients (ICD-10 J44) with at least one exacerbation in the last year, with their own smartphone and connection to pneumological care.
Goals
- Recognising and treating exacerbations preventively
- Reduce hospitalisations and re-hospitalisations
- Improve the health-related quality of life
- Strengthen coordination between outpatient and inpatient treatment
- Reduce costs in the healthcare system in the long term
Added value
- Medical:
Reduction in exacerbations and hospitalisations, stabilisation of the course of the disease, improved quality of life
- Organisational:
Better cross-sector coordination, relief for medical structures
- Economical:
Avoidance of cost-intensive inpatient stays, cost-effectiveness, especially for severe COPD
Core elements of the supply
- Daily recording of relevant vital signs (including respiratory rate, heart rate, oxygen saturation, FEV1, activity)
- Use of validated sensor technology and a GDPR-compliant app
- Algorithm-based traffic light system for risk stratification
- Combination of vital parameters and patient-reported symptoms (COPD Assessment Test)
- Early identification of clinical deterioration
- Continuous review of monitoring data by medical professionals
- Structured telephone or video-based interventions in the event of anomalies
- Coordination of prompt appointments with pneumologists or clinics
- Training and information programmes on the disease, therapy and inhalation technique
- Movement, breathing and motivation training to promote adherence and activity
Evidence
The Telementor-COPD study, funded by the G-BA Innovation Fund, has shown that the care model outlined here - as part of a multicentre, randomised and controlled clinical study under Consortium management of the LungenClinic Großhansdorf - created scientific evidence.
